The aim of this study is a comparative assessment of the immunological parameters of peripheral blood at the cement and cementless hip enoprotezirovanii. The study of immunological parameters of blood of 110 patients with primary hip arthroplasty. Based on the data about long-term outcomes of surgical treatment in terms of up to 5.5 years, all the patients were divided into groups with a favorable (39%) and poor (61%) the result of the treatment. Laboratory tests performed prior to operation, after 3, 10 days, 1, 3, 6, 9 months, 1, 1.5, 2, 3, 3.5 years after the conference. Statistical data processing was carried out using the «Microsoft Office Excel 2007" program and «Statistica for Windows v.6.1». It was found that the implantation of the endoprosthesis is a complex set of immunological reactions caused by a "non-specific" influence of surgical trauma, and "specific" influence of the implant. Informative indicators in immunological monitoring system during hip arthroplasty is the level of lymphocytes and Ig M before the operation, increasing the number of monocytes and reduced Ig M on the third day after surgery.
