24.06.15 больной поступил в отделение трансплантации костного мозга для проведения этапа консолидации в виде высокодозной химиотерапии с аутотрансплантацией ПСК. Статус перед ТКМ - ПЭ.
24.06.15 внутривенно были введены Алло ЦТЛ в дозе 1055*10^6 ( реакция в виде фебрильной лихорадки 2 типа). С 25.06.15 по 30.06.15 проведён курс высокодозной химиотерапии в режиме : Треосульфан 10000 мг/м^2 сут, дни 6,5,4 (СД=30000 мг/м^2); Мелфалан 70 мг/м^2/сут, дни 3,2 (СД=140 мг/м^2). 01.07.15 была выполнена аутотрансплантация ПСК. Клеточность перелитого материала составила : ПСК - 5,0*10^6, СД 34+/кг. На фоне аплазии кроветворения были отмечены проявления инфекционного синдрома в виде фебрильной лихорадки, что потребовало проведения антибактериальной терапии в режиме : меронем, амикацин, метрогил. Противогрибковая терапия - дифлюкан. В связи с явлениями стоматита больной получал парентеральное питание, инфузионную терапию. С заместительной целью проводились трансфузии эритроцитарной массы ( 1 доза) и тромбоконцентрата ( 4 дозы). Переливались иммуноглобулины в дозе 1000 мг/кг. Восстановление WBC >1*10 9|L отмечено на день +9, Г-КСФ отменён со дня +11. Токсичность ВДХТ: гематологическая - 4 ст., геморрагическая - 4 ст., инфекционная - 2 ст.,диспепсическая - 1 ст. В рамках обеспечения процесса изготовления дентритных противоопухолевых аутологичных вакцин 20.07.15 на сепараторе Cobe Spectra был проведён сеанс лейкофереза в режиме МСН. Получено 70 мл сепарата. ПРоцедура без особенностей.
