Основной диагноз : Постлактационная инволюция, асимметрия и тубулярная деформация молочных желез. Сопутствующий: Диффузная кистозная мастопатия

Поступила с жалобами на неудовлетворенность недостаточным объемом, запустеванием молочных желез, также отмечает опущение и асимметрию молочных желез, возникшие после рождения 4х детей. Из анамнеза - 5 беременностей, из них 4 родов, 3 кесаревых сечения, последний раз перестала кормить 6 месяцев назад, хочет грудь 2го размера. После окончания кормления грудь постепенно уменьшилась, верхние склоны стали «пустыми», молочные железы опустились, с более заметной асимметрией МЖ. Желает увеличить объем молочных желез, устранить перечисленные выше деформации при помощи хирургической операции (установка имплантатов).

Операция: 15.09.2016г. 8.40-14.40: Эндопротезирование тубулярных молочных желез по Дель Уэрро с периареолярной коррекцией справа (хирургическое лечение атрофии молочных желез).

После операции: Стол № 7, антибактериальная терапия (цефазолин 1,0 г 3 раза в сутки), так же получала обезболивающие препараты (артрозилен, трамал), гемостатическую терапию (дицинон 500 мг), противовоспалительные препараты (атаракс, цлебрекс) противорвотные препараты (лотран), перевязки. После операции предьявляла жалобы на боли в области левой молочной железы. По дренажам 30мл справа, 75мл слева. Для исключения гематомы в послеоперационной области выполнено УЗИ молочных желез. Патологии не выявлено. Дренажи удалены ( правый - 16.09.16г.., левый – 17.09.16г). Выписывается в удовлетворительном состоянии под наблюдение лечащего врача.

Рекомендовано: Обязательно:
1. Бессолевая диета в течение 2 недель.
2. Прием антибиотиков – Амоксиклав (аугментин) в таблетках 1000 мг 2 раза в день внутрь 5 дней, начиная с вечера 2.06.16г (принимать одновременно Линекс с целью профилактики дисбактериоза по 2 таблетки 3 раза в день 10 дней).
3. Обезболивающие – Катадалон 100 мг до 4 раз в день – при болях.
4. Противовоспалительные препараты - Целебрекс 200 мг по 1 капсуле 2 раза в день, 14 дней.
5. Ношение компрессионного белья 1,5 мес.
6. Ограничение физических нагрузок (не отводить плечи от туловища более чем на 45* в течении первой недели после операции, не поднимать руки выше плеч 2 недели после операции, затем не поднимать руки выше головы 2 недели, не выполнять вращательные движения в плечевых суставах 6 недель после операции, не поднимать тяжести более 3 кг в течение 1 мес.).
7. Не применять аспирин и препараты, содержащие его, в течение 2 недель.
8. Осмотр, перевязка 20.09.2016г. (снятие пластырей ОмниФикс).
9. Ношение стрипов (ОмниСтрип, Стери-Стрип) в области рубцов в течение 3х месяцев.
10. Избегать прямых солнечных лучей в области груди 2 месяца. Пользоваться кремом от загара с числом SPF 30 и более.
11. Плановые осмотры в 1, 3, 6 и 12 месяцев после операции.
12. В связи с продолжением лактации (выявленно интраоперационно), рекомендовано консультация маммолога/гинеколога для решения вопроса о назначении препаратов, подавляющих лактацию (бромокриптин).
