1.Крепление груза.При погрузке плит, были не соблюдены требования правил, процедур и инструкций, а именно:
1.1 Железобетонные плиты были уложены штабелями без специальных деревянных прокладок, что в свою очередь повлекло их дальнейшее скольжение.
1.2.Железобетонные плиты были закреплены с помощью двух крепежных 5 тонных ремней с храповым механизмом, что не соответствует тоннажу перевозимого груза, крепление данных железобетонных плит должно было осуществляться с помощью цепей.Требование п.5.5 Безопасное крепление грузов

1.3. Отсутствовали боковые бортовые стойки трала.
1.4. Площадка трала не была обработана перед погрузкой от наледи.
